According to the report, the two inside the plane were on a training flight. One witness told authorities that whoever was piloting the plane attempted to land the plane two times at the Lakeway Airpark.

It was on the third attempt where the pilot of the plane missed the runway crashed about 150 yards from the runway — just missing a playground and neighborhood swimming pool.

No one on the ground was hurt, but Nelson was taken to an area hospital in serious condition. Henderson was also taken, but later died from his injuries.

The report goes on to note that no mechanical issues were found when investigators took a closer look at the plane after the crash.

The plane was registered to a private individual and was operated by the instructor as part of the Lakeway Flying Club, Inc.
